Who Can Apply:
- Be a legal resident of the United States (U.S.)
- Have physician-documented, active epilepsy seizures within the previous 12 months as of the application deadline, not be a candidate for surgery, and be currently on medication to treat epilepsy.
- High school senior, high school graduate (including GED), or current college undergraduate
- Have not previously earned a bachelor’s degree, unless the applicant is seeking to enroll in a trade or vocational school program
- Plan to enroll in full-time or part-time* undergraduate study at an accredited two-year or four-year college, university, or vocational-technical school in the U.S. for the entire upcoming academic year.
Submission:
- A Physician Diagnosis Verification form completed by your physician.
- A copy of the first two pages of the most recently filed IRS Form 1040 used for the Financial Information section of the application. Please black out all social security numbers and bank account information.
